Falls are a leading cause of injury among older adults, but they don’t have to be inevitable. The Fall Prevention Program focuses on providing education, tools, and resources to help individuals and families take proactive steps to prevent falls. From in-home safety assessments to modification installation, we’re here to ensure everyone can move confidently and securely.

Program Requirements
Senior Age 60+ or Disabled
Resident of Solano County
Fall Risk: Fear of falling, history of falls, or a desire to prevent falls
Program Benefits
In-home Fall Risk Assessment by one of our Licensed Occupational Therapists
Home Safety Evaluation and Modification Plan
Home modifications as well as assistive device installation by a licensed contractor, as needed
Community Resource Referrals
Our Process
Step 1
Request Fall Prevention Assistance through our online form below
Step 2
One of our Fall Prevention Coordinators will contact you to provide more details on our program and eligibilty
Step 3
Occupational Therapist will come to your home for a Fall Risk Assessment and provide a Home Modification Plan
Step 4
Installation of assistive devices as needed and a follow up visit with your licensed Occupational Therapist to educate on safe use of equipment and answer any additional questions.
